PREPARING FOR DETOX
Thank you for considering us to be a part of your recovery journey! While we would love to be a part of everyone’s plan, we understand that you may be concerned about costs of services. We are currently able to work with most individual’s insurance, however, we are unfortunately unable to work with 1199 and Medicare insurance. We will check your benefits after your phone screening, prior to your first appointment and, if there is any cost to you, we will reach out to you by phone to share the results.
Before reaching out to us, please understand that your safety and comfort are of utmost concern to us. In order to be considered for outpatient detoxification services, you must create a plan for the following, if applicable: time off work/school (if you do safety-sensitive work), transportation (as you will not be allowed to drive during detox - if you have Medicaid insurance - a taxi can be arranged for transportation), and childcare. The length of detox is unable to be made without medical evaluations, however, most detoxes (depending on substance) are anywhere from 3-30 days. Ready to connect with us for outpatient detoxification and/or aftercare services? To connect with our Admissions Coordinator, to go through the phone screening to determine level of care needs, please call 212-779-9207 or email them at admissions@parallaxcenter.com. If it seems like our program would be an appropriate level of care, you will then be scheduled for an in-person evaluation with our clinical and/or medical teams. A final determination for admittance into the programs will be made at this appointment.
REFERRING A PATIENT?
Thank you for considering us as you refer your patient for outpatient services! We are currently able to work with most individual’s insurance, however, we are unfortunately unable to work with 1199 and Medicare insurance. We will provide them with the cost of services, by phone, prior to their first appointment.
For Outpatient Detoxification Services
To refer an individual to us for outpatient detox services, please call us at 212-779-9207. If possible, it is best to call us with the individual present. We understand how crucial this time can be and will try to take the call with them, at that time, to go through a phone screening to be assessed for level of care needs. If our program seems appropriate at this time, we will schedule them for an in-person evaluation with our clinical and medical teams. If an alternate level of care is needed, we will provide them with referrals to a program we believe would better meet their current needs and our current capabilities.
For Outpatient Rehabilitation Services
The best way to do this is to first send over their Face Sheet, Medications List, BioPsychosocial, Medical Evaluation, signed HIPAA consent, and Discharge plan if available (a partial would be accepted as well, if possible). You can do this by fax at 646-786-4128 or by email at admissions@parallaxcenter.com.
After this is sent over, please give us a call at 212-779-9207 to schedule a time for the patient to go through a phone screening with the Admissions Coordinator. Depending on the time you call and the patient's discharge date, you would likely be able to schedule the call for later that day or within the next business day.
The Admissions Coordinator will then reach out to you at the number you provide to connect with you and the patient and go through the phone screening with them to determine level of care needs. If our level of care seems to be appropriate for the patient, they will then be scheduled for their in-person evaluation appointment. A final determination of admittance into the program is made at the evaluation appointment. Prior to leaving, if the patient is admitted, they will have their individualized treatment schedule and, regardless of admittance, access to various community referrals, available upon request.
RETURNING PATIENTS
We understand the road to recovery is not a straight line. We are not here to judge you in your time of need. If you are seeking re-entry into our outpatient detoxification and/or aftercare services programs, we would be happy to re-evaluate level of care needs to determine if our program is still an appropriate level of care for you.
